Personalized Social Work Support
ElderONE’s social work services provide participants with the support they need while giving them the independence they desire.
As a vital member of our interdisciplinary team, our social workers are true participant advocates within the program, assuring each participant has their voices heard and their needs met. The primary role as the team social worker is to manage the ongoing relationship between the participant (and their families) and the rest of the care team at ElderONE.
Social work services may include:
- Assessment of social, medical, psychosocial, and spiritual needs
- Individual and family counseling
- Assistance with daily needs (paying bills, home management)
- Assistance with community support services (food, finances)
Our compassionate social workers have decades of experience working with seniors and are dedicated to helping our participants and their families identify and achieve their goals with ElderONE. Our staff empowers participants and families by providing resources and advocacy based on their strengths and needs.
