Our Care Team

An Interdisciplinary Team

When caring for our participants in the community, ElderONE operates as an 11-person interdisciplinary team – all of whom are specially trained in the aging process. Working in synergy, our multifaceted team of dedicated health care professionals help each participant through the continuum of care, keeping our participant’s safety and well-being at the center of all we do.

Working Together for Your Individual Needs

Our ElderONE care team assesses the needs of every participant - working alongside them and their families to develop an individualized care plan. As care needs evolve, the care team delivers and monitors all services, and adjusts the care plan as needed. For the convenience of the participant, care can be provided in a variety of settings— in the home, at an ElderONE PACE day center, and in a nursing home or a hospital.

Our experienced interdisciplinary team of professionals consists of:

  • board-certified geriatricians
  • registered nurses
  • licensed social workers
  • licensed therapists (physical, occupational, recreational)
  • PACE day center managers
  • registered dietitians
  • care aides (personal, home health)
  • professional drivers
  • chaplains

ElderONE's interdisciplinary team meets regularly with participants to ensure that their comprehensive medical and social needs are not only being met but also adjusted after any change in status. Behind the scenes, our care teams meet daily to discuss any changes to their medical, functional, and psychosocial status – helping our participants stay out of the hospital.
