A collaborative approach to caring for seniors.
Our goal is to help our participants remain in their home for as long as possible. To do so, our ElderONE interdisciplinary team works with participants and their families to develop an individualized care plan, carefully selecting the services needed to maintain independence, dignity, and quality of life.
Additional PACE program benefits include:
- Consumer Directed Personal Assistance Program (CDPAP/CDPAS)
- Community First Choice Option (CFCO)
- Diabetes Prevention
Other services are provided as deemed necessary by a team of health care professionals to improve and maintain the overall health of PACE participants.
